The CareStory app leverages QR code technologies on mobile devices to provide individualized, holistic, person-centered care for residents and just-in-time performance support for carers. Each resident’s profile has a quick fact page, personalized music and life history video uploaded by family & caregivers. Upon scanning the secure QR outside the resident’s room, the resident profile page displays.
